Important Update: New No-Show, Cancellation, and Late Policy

Dear Form Orthodontics Patients and Parents,

We hope this message finds you well. We are writing to inform you of an important update to our appointment policies here at Form Orthodontics.

No-Show and Cancellation Policy:
Beginning in January 2025, we will implement a fee of $25 for missed appointments or cancellations made less than 24 hours in advance. This policy is designed to help us manage our schedule more effectively and ensure that all patients have the opportunity to receive timely care.

Late Policy:
Additionally, to maintain our schedule and ensure that all patients receive the care they need on time, we ask that you arrive promptly for your appointment. If you arrive more than 15 minutes late, we will need to reschedule your appointment. This allows us to stay on track and provide the best care possible for all our patients.

We understand that unforeseen circumstances can arise, and we will do our best to accommodate any necessary changes to your appointment. However, to maintain efficient scheduling and minimize disruptions, we kindly ask for your cooperation in adhering to these policies.
